Jewish Family Services (JFS) care managers are gerontology experts, committed to providing excellent service delivery and care coordination that maintains the dignity and achieves the goals of older adults served. Care managers offer thoughtful advice and compassionate support to help older adults maintain or improve their quality of life. Service options are individualized and could be a one-time discussion/consultation or regular, ongoing support.
How does Care Management work?
Jewish Family Service care managers conduct in-home assessments of each person’s strengths and needs. They develop a comprehensive, individualized care plan designed to address those needs and maintain regular contact with family members, near or far, when appropriate.
Care managers coordinate and monitor in-home services, provide healthcare advocacy, assist with long-term care planning, and give information, referrals, and assistance connecting to additional community resources. JFS care managers are also available to provide consultation services by creating a proactive plan for individuals, couples, and families in an effort to ease the stress of aging and planning for the future.
Our team helps older adults maintain as much independence as possible, while preparing for the future. We work alongside our clients to develop a plan of care that ensures opportunities for self-sufficiency, support, and dignity. We make connections to local and national resources for home care, healthcare, mental health support, nursing, transportation, and safety.
